Dr Andy Phillips
Regional Executive Officer, HSE South West 

A Welshman, Dr Andy Phillips comes to the HSE from his role as Acting CEO of Safer Care Victoria, Australia. He has worked in clinical, operational and strategic executive leadership roles in healthcare for the past 37 years. Andy maintains his clinical qualification as a consultant clinical scientist. 

Since 2009, Andy has been leading large scale health system transformation in Wales, New Zealand and Australia, with a passion for integrating health and social care. Andy has practised co-creating health and relationship centred healthcare for over 20 years, with recent work leading kindness and compassion in healthcare. Andy took up his role as Regional Executive Officer in HSE South West on 1 March 2024. 

HSE South West serves the people of Cork and Kerry with a purpose of making our communities proud of their Health Service

Rupa Valdez
Professor of systems and information engineering and public health sciences at the University of Virginia, USA

​​​Her work focuses on understanding and designing solutions to support the ways in which people manage health at home and in the community, drawing on methods from human factors engineering, cultural anthropology, and health informatics. She received the Jack A. Kraft innovator award from the Human Factors and Ergonomics Society in the U.S. for her pioneering work to establish the field of patient ergonomics. She serves as Associate Editor for Ergonomics and the Journal of American Medical Informatics Association Open.

Dr Shanqing “SQ” Yin
Professor at Duke-NUS Medical School and the Human Factors Consultant for Singapore Health Services

​​As Singapore’s first embedded HF/E specialist, Dr. Yin has spearheaded the integration of human factors science into the nation’s public health systems for over thirteen years. He is also a recognized subject matter expert for the World Health Organization’s various patient safety initiatives. As an academic leader, he directs the faculty development framework for clinical educators, and advances scholarship in human factors and patient safety. He serves as a Senior Editor for Ergonomics in Design.

Maureen Nolan
Member of Patients for Patient Safety Ireland (PFPSI)

Maureen Nolan is a member of Patients for Patient Safety Ireland (PFPSI), since its formation in January 2013 under a World Health Organisation programme. PFPSI Champions sit at many HSE service development and quality improvement tables as full and equal partners and advocate for person centred care through collaboration with all stake holders, including the patient across all levels of policy development, implementation, evaluation, design and delivery of services.

Enid Montague
PhD, Canada Research Chair in human-centred automation at the University of Toronto department of Mechanical and Industrial Engineering

Enid researches appropriate automation between physicians, patients and technologies and designing systems for health equity, and patient safety. She uses mixed methods in naturalistic settings to model human technology interaction and develop guidelines for human-automation that is efficient, effective and safe for patients and health care providers.

Mental health advocacy and community health advocacy for ethnics and minority group.

Aoibhinn Walsh
Consultant Paediatrician 

Aoibhinn is a Consultant Paediatrician with a special interest in Inclusion Health. She is the
principal investigator and lead clinical researcher of the Lynn Clinic which looks at inclusive and equitable healthcare delivery to Paediatric populations experiencing social adversity e.g., children who are homeless, refugees and international protection applicants, and ethnic minorities who experience difficulty with engaging with healthcare services.
 
Aoibhinn has always had an interest in healthcare delivery to marginalised children which was strengthened when she went overseas with Médecins Sans Frontières to work as an expatriate Paediatrician. Her current clinical focus is on the establishment and future planning of a Paediatric Inclusion Health service within Children’s Health Ireland (CHI) and the HSE to formalise care pathways for children at risk of social exclusion with a strong focus on a robust system of care between hospital and community health and social care practitioners

Dr. John Sheehan
Radiologist, Blackrock Health

​John is a Consultant Radiologist whose expertise transcends medical imaging, reaching into the vanguard of digital health innovation and transformative healthcare leadership. With fellowships at Northwestern Memorial Hospital in Chicago, John's clinical acumen is matched by his strategic roles at Blackrock Health, Dublin, Mobile Medical Diagnostics (MMD), and xWave Technologies. His leadership roles as Clinical Director at MMD, Clinical Advisor at xWave, and Vice Chair of Ireland's Digital Health Association, highlight his commitment to AI-driven diagnostic advancements and the betterment of community healthcare. John champions a healthcare ethos centered on accessibility, equity, and integration. His contributions to MMD and xWave are a testament to his dedication to leveraging digital innovations to remove patient care barriers and drive transformative change.

Tiberius Pereira
Co-Founder Patients for Patient Safety Ireland

Tiberius Pereira is a co-founder of Patients for Patient Safety Ireland (PFPSI), formed 11 years ago under a World Health Organisation programme. As defined by the WHO, PFPSI aims to engage and empower patients and families to be active partners and advocates in their own care. The aim is to work in partnership and collaboration with health professionals and policy-makers to make health services safer in Ireland and enable the patient voice to be heard fully https://www.who.int/initiatives/patients-for-patient-safety).

Karen Thomson
Patient Safety Specialist Investigator at an NHS Trust, in the West-Midlands, England

Her work focuses on supporting the NHS with developing and maintaining effective systems and processes for responding to patient safety incidents.  Karen has worked with NHS Trusts to implement the Patient Safety Incident Framework (PSIRF), which signifies a significant shift in the way the NHS responds to safety incidents.  With over a decade experience in Patient Safety, both locally and internationally, she is a keen advocate for compassionate engagement and involvement with those affected by patient safety incidents, which are key components of PSIRF.

Dr. Megan Salwei
Assistant Professor in the Center for Research and Innovation in Systems Safety (CRISS) in the Department of Anesthesiology at Vanderbilt University Medical Center in Nashville, Tennessee

She holds a secondary appointment in the Department of Biomedical Informatics and is a member of the Vanderbilt Clinical Informatics Center (VCLIC) and the Artificial Intelligence Discovery and Vigilance to Accelerate Innovation and Clinical Excellence (ADVANCE) Center. In her research, she applies human factors engineering methods and principles to improve the design of health information technology (IT) to better support clinicians and patients, and to enhance patient safety and care quality.

Prof. Clíona Ní Cheallaigh
Associate Professor in the Department of Clinical Medicine

Professor Clíona Ní Cheallaigh is an Associate Professor in the Department of Clinical Medicine, TCD and a Consultant in Infectious Diseases and General Medicine in St James’s Hospital, Dublin. She is the Clinical Lead of the Inclusion Health Service in St James’s Hospital. Her research examines the effect of social exclusion on health, including work on health systems design and evaluation, premature ageing in homeless adults and the effect of social exclusion on the immune system.

Dr. Adeola Bamgboje-Ayodele
Associate Professor in the DepartResearch Fellow in Human Factors at the University of Sydney, Australiament of Clinical Medicine

​She has led high-impact human factors research on designing, evaluating, and implementing digital technologies to improve healthcare delivery. She serves as Associate Editor for JMIR Human Factors. She is interested in optimising patient-work for older people who use virtual care technologies.
